It's the extra "cheater" button by itself on the fourth row, added by the Slovenian accordion legend Lojze Slak (1932-2011).

On a C/F/Bb it plays Eb in and C# out, which is very convenient when playing on the Bb row, and is considerd to add to the versatility of the instrument.
