If you missed this programme on BBC2 (I think) - shame on you if you are a Londoner - it has some melodeons in it Mawkin with Eliza Carthy and Spiers and Boden plus some archive footage, one clip of particular interest.  It is of a busker playing a box (no sound if I recall) but the bass end is held in place over the shoulder by a strap and the treble end is pumped in and out and played at the same time (is this what the thumb strap was invented for?)

There is a chap who plays (played?) for Crooke Morris who plays a melodeon like that, due to an injury to his left arm. It is quite disturbing to watch, but even more disturbing to try and play it.
